The Rebreather unlocks every underwater nook on the Island, which sounds great until you realize the game hides it behind a dark cave, some hungry mutants, and at least one annoyed shark. Here is the cleanest route to grab it, plus the gear you should bring before walking in.
For context, the Sons of the Forest Rebreather is purely a cave reward. No crafting recipe involved, no boss fight needed.

Head to the northern coastline of the map. A handful of starting spawns drop you almost on top of the area, so check your surroundings before sprinting across the whole Island.
Once you reach the shore, look along the beach for a cave entrance flanked by two yellow barrels. Those drums are the giveaway. If they are missing, you are standing in front of the wrong cave.
The mouth of the cave is sealed with wooden planks. Open your inventory, equip the axe, and chop the boards down. From here, the dungeon begins.

If you went for the Rebreather early, the Flashlight is probably still on your wishlist. No problem. A torch will do the job:
That improvised torch is enough to read the layout of the cave without faceplanting into a wall.
Push forward until the ceiling drops. Crouch through the squeeze and you will pop into the next chamber.
The corridor ahead leads past a rubber floater and into three mutants on patrol. If you have decent supplies, fight them out. If you walked in light, run, soak the first hits, and keep moving. The Rebreather room is close enough that pushing past is a valid plan.

After clearing or skipping the trio, you will arrive at a split.
A shark is in there. If curiosity wins and you dive in, you will get bitten. The good news: you do not need to fight it to finish the run.
Look for a narrow side path along the room. Follow it to the end and the Rebreather is yours.
Tip: Equip the Rebreather on the spot and use the underwater tunnel right next to it to leave the cave. It is the fastest exit, and it skips the entire combat section on the way back.
